1、Legal applicants: biological parents, grandparents, head of the household or legal proxy.

2、Required documents: Birth Certificate (with the agreement of baby’s surname by parents’ signatures), applicant’s seal, ID Card, Household registry, Letter of Legal Proxy.

3、For children born in foreign countries (including Mainland China), applicant must first apply for a Permanent Resident Certificate for the child from the National Immigration Agency, after which application can be made for initial Household.
